Your eye appointment:
Our office hours are Monday through Friday, 8:00 am to 5:00 pm. When you call our office at 312-475-1000, we will schedule your appointment and will be happy to answer any questions you have. Please arrive 15 minutes prior to your scheduled appointment to allow time to complete medical history and office forms.
As a new patient, you will receive a thorough eye examination which includes dilation of your pupils. Please plan on being in our office for at least one-and-a-half hours. Your examination may be longer depending on the severity of your condition and other specialized testing. We suggest bringing a pair sunglasses and, if possible, someone to drive you home. Your vision might be blurred for several hours after pupil dilation.
Eyeglasses and Contact lens services:
We have our own optical shop that is staffed by our Certified Optician, Dawn Haldiman, to help you with your eyeglass selection and contact lenses. We have a wide assortment of frames from which to choose. Sunglasses, specialized lens coatings, no-line bifocals, and tinted lenses are all available in our office.
All new contact lens fittings are done in our office by our optician. We can provide both disposable and daily-wear soft contact lenses, soft lenses that correct asitgmatism, and gas-permeable lenses. If you are a new contact lens wearer, or if you should require a lens fitting for a new type of contact lens, then a separate appointment is required. New contact lens fittings also have a charge separate from an eye examination.
Fee for services:
University Eye Specialists are providers for many different insurance companies. We will be happy to assist in submitting all insurance claims to your health insurance provider. Please bring your insurance cards with you at time of your visit. Also, the doctors at University Eye Specialists accept Medicare assignment.
The cost for a refraction (measurement for glasses) is not covered by Medicare and most other insurance carriers. The doctors’ fee for this service is $26 and is only charged when a new prescription for glasses or contact lenses is given to you.
If you belong to a Managed Care Health Care program and you are required to have an Insurance Referral / Authorization Form from your Primary Care Physician, or appropriate network source, please be sure that you bring it with you at the time of your visit. Without proper authorization, the doctor will not be able to see you at your appointed time and you will be held responsible for the cost of your examination. |
